Description
American Traveler is hiring an RN Case Manager for an acute inpatient hospital setting requiring NJ or compact license and discharge planning experience.
Details- Acute care hospital setting within a Care Management department
- Coordinates patients through acute care hospitalization and discharge planning
- Manages transition process for subacute and rehabilitation placement patients
- Performs utilization management and quality screenings
- Day shift schedule from 8:00 AM to 4:00 PM, 37.5 hours per week
- Rotating weekends required every 4th or 5th weekend
- Holiday coverage required for one summer and one winter holiday
- May float to sister hospital as needed
- No on-call responsibilities
- Uses Epic for charting
- Active NJ or compact RN license required
- BSN required
- CCMC certification preferred
- Minimum 2 years of acute inpatient hospital discharge planning experience required
- Hospital experience including discharge planning and rehabilitation facility placement required
- Experience with subacute patients required
- In-patient case management experience required
- Epic EMR experience preferred
- Coordinates all aspects of patient discharge planning and care transitions
- Collaborates with care team on utilization management and quality screening processes
- Local candidates accepted
- First-time travelers considered if experience requirements are met
- Travelers may remain on assignment up to one calendar year from initial start date, with a required break before returning to a new assignment
