Description
American Traveler is seeking a compassionate and skilled Physical Therapist for a 13-week contract at a Skilled Nursing Facility, with experience in SNF or geriatric rehab settings preferred.
Details- Skilled Nursing Facility (SNF) setting, focused on inpatient, long-term care, and short-term rehabilitation
- Patient population includes geriatric residents requiring mobility restoration, functional improvement, and independence optimization
- Day shift schedule: 9:00 AM – 5:00 PM
- 13-week contract
- Graduate of an accredited Physical Therapy program required
- Current, active state PT license required; license required if applicable
- CPR/BLS certification preferred
- Experience in a SNF or geriatric rehab setting preferred
- Strong communication, documentation, and interpersonal skills required
- Ability to perform physical tasks including lifting, standing, walking, and assisting patients with transfers and mobility
- DOB and SSN may be required for consideration
- Responsibilities include evaluating residents' physical conditions, developing individualized treatment plans, and delivering evidence-based rehabilitative interventions
- Collaborate with an interdisciplinary team including physicians, nursing staff, occupational and speech therapists, and family members
- Monitor and document patient progress in accordance with state, federal, and Medicare/Medicaid guidelines
- Educate patients and caregivers on safe mobility techniques, home exercise programs, and adaptive equipment use
- Supervise physical therapist assistants (PTAs) and aides as needed
- Participate in care planning meetings and interdisciplinary team conferences
