Description
American Traveler is seeking a Home Health Physical Therapist with 1+ years of recent experience and HCHB charting experience for a geriatric patient caseload.
Details- Home Health setting serving a geriatric patient population
- Average caseload of 5-7 patients per day
- Day shift schedule, 5x8 hours from 8:00 AM to 5:00 PM
- No on-call requirement per facility details, though on-call may be required per compliance checklist
- Covers a territory up to a 60-mile radius from the hub/office and requires flexibility with territory
- Uses HomeCare HomeBase (HCHB) for charting
- iPad provided for documentation; cell phone not provided
- Mileage reimbursed at the IRS standard rate
- Minimum 1 year of recent Home Health experience required
- Current BLS certification required
- OASIS experience required for clinicians performing start of care (SOC) visits
- HCHB charting experience required
- License required if applicable
- License must be in hand, with pending license accepted for therapy positions only
- Proof of vehicle insurance required
- Scrubs required (any color, no jeans)
- Main responsibilities include providing physical therapy services to geriatric patients in a home health setting
- Open to first-time travelers
- Local candidates accepted at the travel rate
- Traveler must be flexible with territory coverage across the assigned radius
