- Monday to Friday
- 5-6 patients per day
- 13 - week contract assignment with option to extend
- Oasis Experienced required within the last year
- The RN performs the SOC (Start of Care-opening the case) and Discharges (closing the case) and the LPN does the revisits in between, however, our RN must know that they will also be needed to perform Revisits when necessary.
- RN will also perform Supervisory Visits every 30 days (with the LPN’s)
Local hourly rate: $65/hr - we pay per hour not per visit!
Must have recent home health experience.Must have recent OASIS experience within the last year.
