Description
American Traveler is seeking a licensed Physical Therapist with Home Health experience and OASIS proficiency for a 13-week travel assignment in Alameda or Contra Costa County, CA.
Details- Home health setting serving patients in Alameda or Contra Costa County, CA
- Day shift schedule, 5x8-hour shifts (9:00 AM – 5:00 PM)
- Weekends required on an as-needed basis
- No on-call requirement
- Uses Kinnser/WellSky EMR; training provided
- Minimum 1 year of recent home health PT experience required
- OASIS experience required
- Active CA PT license required; license must be in hand at time of consideration
- Current BLS certification required
- Responsibilities include physical therapy evaluation and treatment of patients in a home health setting across Alameda or Contra Costa County
- 2 professional references from a supervisor within the past 2 years required for consideration
- Full 7-year work history with all gaps explained required for consideration
- All time-off and scheduling requests must be submitted at the time of initial consideration
- License required if applicable
