Travel Physical Therapist

Type: Three-month contract, with possibility of an extensionSchedule: Monday – Friday | 8:00 AM – 4:30 PM
Location: Selinsgrove, PA 17870
Pay: $56 to $59 per hour
Primary Responsibilities:- Complete start of care visits, physical therapy evaluations, and routine home health visits
- Assess and evaluate patient care needs related to mobility, endurance, pain, and functional status
- Develop and implement individualized treatment plans for home health patients
- Perform therapeutic interventions to improve patient strength, mobility, and overall well-being
- Monitor patient progress and adjust treatment plans as needed
- Document patient care using Home Care Home Base (HCHB) electronic medical records
- Collaborate with physicians, nurses, caregivers, and interdisciplinary healthcare teams
- Travel between patient homes within assigned coverage areas in Lycoming County, PA
- Maintain compliance with company policies, state regulations, and patient care standards
- Active Physical Therapist license required
- Previous Home Health experience required
- Experience using Home Care Home Base (HCHB) EMR preferred
- Strong communication and patient care skills
- Prior experience with LHC Group or similar home health settings preferred
- Business casual or blue scrubs
Available paid leave may include Paid Sick Leave, where required by law; any other paid leave required by Federal, State, or local law; and Holiday pay upon meeting eligibility criteria.

Pay Details: $56.00 to $59.00 per hour
