- Shift: 8-hour days, Monday - Friday, 7:00 AM - 3:30 PM

- Unit Coverage: 34-38 patients per unit, typical 1 RN : 25 patient ratio
- Start Date: May 4, 2026
- Minimum 5 years of registered nurse (RN) experience, with demonstrated case management or care coordination experience
- BSN required
- Strong clinical knowledge in inpatient nursing, care coordination, and discharge planning
- Excellent communication, organization, and interdisciplinary collaboration skills
- Critical thinking, problem-solving, and patient advocacy skills
- Attention to detail, empathy, and a patient-centered approach
- Familiarity with Midas documentation system or similar electronic medical record systems preferred
- Manage caseloads of 34-38 patients per unit, ensuring high-quality case management and care coordination
- Perform discharge planning, utilization review, and patient advocacy to support safe transitions of care
- Utilize the Midas documentation system for accurate record-keeping and regulatory compliance
- Collaborate closely with Case Management Assistants, physicians, nurses, social workers, and other healthcare professionals
- Float between units as needed and adapt to department-specific coverage and workflows
- Apply critical thinking and problem-solving skills to address complex patient needs and barriers to care
- Maintain a compassionate, patient-centered approach in all interactions and ensure clear interdisciplinary communication
