Your daily duties will include assisting with personal care, facilitating community involvement, and implementing individualized support plans. Responsibilities- Assist individuals with daily living activities including hygiene, meal preparation, and medication management.
- Support clients in community integration and social engagement.
- Implement individualized care plans and accurately record client progress.
- Ensure safety and well-being of clients at all times.
- Collaborate with families and multidisciplinary teams to optimize care outcomes.
- High school diploma or equivalent required.
- Previous experience in direct support or related caregiving roles preferred.
- Excellent communication and interpersonal skills.
- Ability to work flexible hours, including weekends and evenings.
- Valid driver’s license and reliable transportation.
- CPR and First Aid certification preferred or required to obtain upon hire.
