Travel Physical Therapist (PT) – Home Health

Medford, New York

We are seeking a Travel Physical Therapist for a Home Health contract in Medford, New York. This opportunity offers the independence and flexibility of home health while providing meaningful one-on-one care to adult and geriatric patients in their homes.

• Perform comprehensive physical therapy evaluations and reassessments.
• Develop individualized treatment plans based on physician orders and patient goals.
• Treat orthopedic, neurological, post-surgical, cardiopulmonary, and medically complex patients.
• Provide gait training, balance training, transfer training, therapeutic exercise, fall prevention, and functional mobility interventions.
• Educate patients and caregivers on home exercise programs, safety strategies, and proper equipment use.
• Collaborate with nurses, physicians, case managers, and interdisciplinary care teams.
• Complete timely documentation and maintain home health compliance standards.
