Travel PPV LPN
This is an on-site, full-time contract position providing in-home nursing services across the assigned coverage area. Apply with a current resume for consideration.
Assignment Details:
Pay: $32 to $34/hrLocation: Abbeville, LA 70510
Contract Duration: 05/18/2026 – 08/15/2026 (with possibility of extension)
Weekly Hour Guarantee: 40 hours
Schedule: Monday – Friday, 8:00 AM – 4:30 PMOn-Call/Weekend Requirement: 1 weekend per month (Saturday & Sunday, 8:00 AM – 4:30 PM)
Coverage Area: Abbeville and Kaplan
Mileage: Reimbursed between patient visits (from first patient to last patient)
Job Summary:
The Travel LPN will provide skilled, patient-centered home health nursing care, supporting patients in managing their medical conditions within the home setting. This role involves delivering direct patient care, coordinating with clinical teams, and ensuring compliance with home health standards and documentation requirements.
Primary Responsibilities:
- Provide direct patient care in a home health setting
- Monitor patient condition and report changes to the care team
- Administer medications and treatments as prescribed
- Complete documentation in accordance with agency and regulatory standards
- Collaborate with RNs, physicians, and interdisciplinary care teams
- Educate patients and caregivers on care plans and disease management
- Travel throughout assigned coverage areas to provide in-home care
- Ensure compliance with all home health regulations and policies
- Support overall patient safety and quality of care initiatives
Qualifications:
- Active Licensed Practical Nurse (LPN) license required
- Minimum 1 year of LPN experience required
- Home Health experience required
- Experience with HCHB (Homecare Homebase) required
- Current BLS certification required
- Valid driver’s license, auto insurance, and reliable transportation
- Ability to travel within assigned service area

Pay Details: $32.00 to $34.00 per hour
