Job Summary
We are seeking a dedicated and experienced Home Health & Hospice Registered Nurse (RN) to provide high-quality, patient-centered care in a home setting. The RN will manage a variety of cases, including chronic and acute conditions, while ensuring proper documentation and adherence to care standards.This role requires strong clinical skills, independence, and the ability to adapt to different patient environments.
Key Responsibilities- Provide skilled nursing care to Home Health and Hospice patients
- Perform patient assessments and develop individualized care plans
- Handle admissions (SOC), recertifications, and resumptions of care
- Administer medications, IV therapy, and wound care treatments
- Manage chronic conditions such as CHF, diabetes, and renal disease
- Ensure accurate and timely OASIS documentation
- Educate patients and families on care plans and disease management
- Maintain infection control standards in home settings
- Participate in on-call rotation (evenings and weekends)
- Average of 6 points per day (e.g., 1 admission + 3–4 visits)
- Minimum 1 year of RN experience
- Recent Home Health experience (within last 2 years)
- Strong OASIS documentation proficiency
- Experience with wound care and infection control
- Active RN license (required at submission)
- BLS certification (AHA) required
