The Home Health RN provides skilled nursing care to patients in their homes across multiple counties. This role focuses on delivering high‑quality, patient‑centered home health services, supporting patients with ongoing medical needs, and ensuring safe, coordinated care while working independently in the field.
Schedule & Work Details:
- Weekly Hour Guarantee: 40 hours
- Schedule: Monday through Friday, 8:00 AM to 4:30 PM
- On Call and Weekend Requirement: None
- Coverage Area: Jefferson/Blount County
- Mileage Reimbursement: Yes, reimbursed between patient visits from first patient to last patient
- Pay Rate: $50-53 per hour
Key Responsibilities:
- Case Management
- Oasis Assessment
- Wound Care, Catheter Care, Labs
- Disease Process Education, Medication Education, Picc Line Care and Education
- Lpn And Aide Supervisory Visits
- Team Case Conference Meetings.
Required Qualifications:
- Active RN license
- Current CPR certification
- Ability to work independently in a home health environment
- HCHB/PointClick Care experience
