Job Details- Weekly Estimated Pay: $1,750 – $1,840 (competitive travel nurse pay)
- Location: Gunnison, CO 81230 — Colorado home health jobs / Gunnison travel nursing
- Assignment Duration: 13 weeks (April 27, 2026 – July 27, 2026)
- Hours per Week: 36 guaranteed hours
- Shift: Day shift with call requirements (1–2 nights per week and 1–2 weekends per month)
- Shift Duration: 12-hour rotating
- Employment Type: Travel / Contract — Home Health RN / Travel RN role
- Active RN license (Nursing certification required)
- At least 1 year of clinical experience in home health, hospice, or palliative care nursing (Home Health RN, Hospice RN, or Palliative Care Nurse)
- Colorado Adult Protective Services (CAPS) check required
- Reliable transportation required due to limited public transit in rural Gunnison
- Ability to participate in on-call rotation as scheduled
- Provide skilled nursing care and case management for home health, hospice, and palliative care patients (Home Health Nurse duties)
- Conduct initial home health evaluation visits and perform ongoing patient assessments
- Develop, implement, and revise individualized Plans of Care and coordinate interdisciplinary care
- Administer preventive and rehabilitative nursing procedures and medication administration as appropriate
- Prepare clinical and progress notes; coordinate services and communicate effectively with physicians and care teams
- Counsel patients and families on nursing needs, symptom management, and end-of-life care when applicable
- Participate in in-service training and provide supervision/teaching to other nursing personnel
