Description
American Traveler is seeking an experienced Home Health Physical Therapist with a minimum of 3 years of experience and HCHB proficiency for a 12-week day shift contract.
Details- Home health setting serving an adult to geriatric patient population
- Common diagnoses include joint replacements, COPD, CHF, and T2DM
- Expected caseload of 6 visits per day with an average daily census of 175
- HCHB EMR experience is required
- Day shift schedule: Monday–Friday, 8:00 AM–4:30 PM (5x8-hour shifts)
- No weekend shifts required
- Floating within the facility and scope of practice as needed
- Minimum 3 years of recent home health PT experience required
- Current BLS certification required
- Active PT license required if applicable
- Proficiency with HCHB EMR system required
- Must have copies of all required certifications available for consideration
- First-time travelers will not be considered for this position
- Local candidates are accepted
- RTO is limited to a maximum of 7 days; RTO cannot be requested during the first week of the assignment
- Holiday coverage is required: if one or two major holidays fall within the assignment period, the traveler is expected to work all of them; if three major holidays fall within the assignment period, the traveler is expected to work two
- Major holidays include Memorial Day, Independence Day, Labor Day, Thanksgiving/Black Friday, Christmas Eve/Christmas Day, and New Year's Eve/New Year's Day
- Two supervisory references (manager level or above; charge nurses not accepted) are highly preferred
- No special scheduling requests, alternate shifts, or schedule changes are permitted
- Travel pairs may not be assigned to the same shift on the same unit
- Travelers must have been away from all facilities in this health system for at least one full year before returning as a traveler
- Any color scrubs are accepted
- Full compliance with onboarding requirements is necessary to begin on time; delays may result in a two-week start date pushback due to bi-weekly orientation scheduling
