Occupational Therapist – Post Acute Care

Hours: 30 Hours/Week Guaranteed

We are seeking a compassionate Occupational Therapist to provide rehabilitation services in a post-acute care environment. This role focuses on helping patients regain independence and improve daily living skills.

• Evaluate patients and develop occupational therapy treatment plans
• Assist patients with improving daily living and functional skills
• Implement therapeutic interventions and adaptive techniques
• Document patient progress and treatment outcomes
• Collaborate with rehabilitation and nursing teams
• Educate patients and caregivers on therapy goals and exercises

• Active Occupational Therapist license in Colorado or compact eligibility required
• Graduate of an accredited Occupational Therapy program required
• Strong interpersonal and patient care skills required

• Previous LTC or rehabilitation experience preferred
• Current BLS certification preferred
