- Active RN license in Pennsylvania (PA RN)
- Minimum 1 year of nursing experience; homecare or home health experience preferred
- Ability to work independently and manage a caseload (case management skills)
- Strong communication and organizational skills
- Valid driver's license and reliable transportation for community/home visits
- Provide skilled nursing care to patients in their homes (home health nursing, homecare)
- Assess patient needs and develop individualized care plans based on clinical findings
- Administer medications, wound care, and other treatments as prescribed
- Collaborate with interdisciplinary team members and home health agency staff to ensure optimal patient outcomes
- Educate patients and families on health management, self-care techniques, and discharge planning
- Document all patient interactions and care provided accurately and in a timely manner
