About the Role:Seeking an experienced Clinical Social Worker to provide psychosocial support, crisis intervention, and discharge planning services within an acute care hospital setting.
Key Responsibilities:
- Conduct psychosocial assessments for patients and families
- Provide counseling, advocacy, and crisis intervention services
- Support patients with mental health, substance abuse, and social needs
- Develop individualized discharge plans
- Coordinate referrals for rehabilitation, long-term care, and community services
- Assist with financial assistance and community resource coordination
- Collaborate with interdisciplinary healthcare teams
- Maintain accurate EHR documentation
- Participate in quality improvement and departmental initiatives
Requirements & Preferred Skills:
- Master’s Degree in Social Work (MSW) required
- Active Massachusetts LCSW or LICSW license required
- CPI/MAB certification required within 1 month of hire
- Strong EHR and computer skills required
- Acute care hospital experience preferred
- Experience with crisis intervention and discharge planning preferred
- Behavioral health and substance abuse counseling experience preferred
- Strong communication, organization, and multitasking skills
- Ability to work independently in fast-paced environments
