- Provide hands-on, personalized care in patients’ homes
- Manage and coordinate a caseload from start to finish
- Perform assessments and develop individualized care plans
- Monitor patient progress and adjust care as needed
- Collaborate with physicians and interdisciplinary teams
- Educate patients and families for long-term success
- Ensure compliance with all clinical and regulatory standards (including HIPAA)
- Active, unrestricted RN license
- 1+ year of clinical nursing experience
- Ability to travel within the coverage area (up to 50%)
- Reliable transportation, valid driver’s license & insurance
- Strong clinical judgment and communication skills
- Ability to lift, move, and assist patients (50–100 lbs intermittently)
Preferred:
- Home Health or Hospice experience
- Experience with HomeCare HomeBase
- ADN or BSN
