Description
American Traveler is hiring an experienced Community Health RN for a home health position in Pittsfield, MA, requiring active MA licensure and certified home health experience including OASIS proficiency.
Details- Home health / community health setting with patient care delivered in the home
- Day shift schedule: 8:00 AM - 4:30 PM, 8-hour shifts, 40 hours per week
- No on-call requirements, though some flexibility with work hours is expected
- No weekend requirements; holiday coverage may be required
- Caseload involves sick and injured patients receiving care at home
- Self-scheduling of caseload may be required
- Active MA RN license required and must be in hand at time of consideration — license will not be pending
- Current AHA BLS certification required
- Minimum one year of acute nursing care experience required; hospital-based experience preferred
- Home health experience required
- Certified Home Health designation required, including proficiency with OASIS
- Infusion Therapy Certification preferred
- BSN highly preferred
- Valid driver's license and personal vehicle with insurance required — candidate must have their own vehicle
- Demonstrated ability to practice independently with minimal supervision
- Strong critical thinking, time management, communication, and problem-solving skills required
- Ability to drive in inclement weather required
- Home Health RN skills checklist (current within 1 year) required for consideration
- One supervisory signed reference from within the past two years of employment required for consideration
- Provides, teaches, supervises, and documents nursing care for patients in the home setting
- Collaborates with healthcare providers, professionals, and community representatives to coordinate patient care
- Evaluates patient and family responses to care interventions and adjusts plans accordingly
- Working knowledge of home care reimbursement and regulatory guidelines is necessary to effectively provide care
- Candidates must reside more than 50 miles from the facility to be eligible for this position
- All RTO requests must be submitted at the time of application — no more than 5 days of RTO will be approved, and requests made after offer or during interview will not be approved
