Registered Nurse – Case Manager

• Coordinate patient care across the continuum to ensure quality outcomes and efficient resource utilization.
• Facilitate discharge planning and transitions of care for patients and families.
• Conduct utilization reviews, case management assessments, and care coordination activities.
• Collaborate with physicians, social workers, nursing staff, and interdisciplinary healthcare teams.
• Monitor patient progress and identify barriers to discharge or treatment plans.
• Ensure compliance with regulatory requirements, payer guidelines, and organizational policies.
• Advocate for patient needs while promoting safe and effective care delivery.
• Maintain accurate and timely documentation of case management activities.
• Support patients and families through education and resource coordination.

• Active Registered Nurse (RN) License.
• Current BLS Certification.
• Recent Case Management experience required.
• Strong knowledge of discharge planning, utilization review, and care coordination.
• Excellent communication, organizational, and problem-solving skills.
• Ability to work collaboratively with multidisciplinary healthcare teams.

• Acute care hospital case management experience.
• Experience with utilization management and payer regulations.
• Familiarity with electronic medical record (EMR) systems.
• Certification in Case Management (CCM or ACM) preferred.
