Job Summary:
Seeking a licensed Occupational Therapist for Home Health visits treating adult med/surg, orthopedic, cardiac, neuro, and post-surgical patients. The OT will provide skilled in-home therapy services and work independently to manage daily patient visits.
Key Responsibilities:
- Conduct occupational therapy home health visits
- Provide ADL training, mobility, and safety training
- Educate patients and caregivers on treatment plans
- Develop home exercise programs (HEP) and splinting as needed
- Complete documentation including OASIS documentation
Requirements:
- Active State Occupational Therapist (OT) License
- Active BLS Certification
- Minimum 1 year of OT experience
- Home Health experience preferred
- Valid driver’s license and reliable vehicle required
- Ability to complete 6–8 patient visits per day
- COVID and Flu compliance required
