Bilingual Case Management Coordinator (Field-Based)
Are you a self-motivated professional with a passion for community health? We are seeking a highly organized and energetic Case Management Coordinator to join our dynamic team. This role offers the perfect blend of remote administrative work and meaningful, face-to-face member engagement across Miami-Dade County.
Position Overview- Pay Rate: $36.07 per hour
- Schedule: Monday – Friday | 8:00 AM – 5:00 PM (Standard Business Hours)
- Location: Work from Home (Remote)
- Travel: 50% – 75% local travel required for face-to-face member visits.
- Training: Conducted remotely via Microsoft Teams.

Key Responsibilities
You will manage a diverse caseload of supportive and medically complex members, serving as their primary advocate and care navigator.- Assessment & Planning: Conduct telephonic and face-to-face assessments to evaluate medical needs and social determinants of health.
- Care Coordination: Determine and authorize appropriate services, including prior authorizations and community resource referrals.
- Clinical Collaboration: Coordinate directly with PCPs, skilled providers, and facility staff (ALFs and Nursing Homes) to ensure seamless care integration.
- Condition Management: Provide education on condition management and perform medication reviews to enhance long-term wellness outcomes.
- Documentation: Maintain precise, tech-savvy records of all interventions and care plans.
- Language: Fluent Bilingual (English/Spanish) is REQUIRED. You must be able to speak, read, and write fluently in both languages.
- Mobility: Must be willing to travel 75% of the time to see members in their homes, assisted living facilities, and nursing homes.
- Technical Skills: Proficient in Microsoft Teams and comfortable navigating various healthcare software platforms.
- Soft Skills: High level of autonomy, detail-oriented, and excellent organizational skills.
- Competitive Pay: Fixed rate of $36.07/hr.
