- Current CPR certification
- Valid driver's license, vehicle insurance, and reliable transportation
- Provide skilled nursing care to patients in their homes across Berwick and nearby Pennsylvania communities
- Perform comprehensive patient assessments and create individualized care plans
- Document patient care accurately using OASIS and HCHB systems
- Work collaboratively with interdisciplinary healthcare teams to improve patient outcomes
- Educate patients and families on home health care management and best practices
- Maintain compliance with all regulatory and clinical standards
