Primary Care Physician We are currently seeking a Primary Care Physician to join other providers in one of our locations as part of the South County Medical Group. Our new physician will see patients within a patient-centered medical home model offering strong collaboration with a pharmacist and nurse case manager to support high-risk patient populations.
The Center allows our physicians to coordinate care with specialists within the group.
