Minimum Requirements- Bachelor's degree in Physical Therapy from an accredited program approved by the American Physical Therapy Association required; Masters degree or Doctorate preferred.
- Current licensure and registration as a Physical Therapist in the Commonwealth of Massachusetts required.
- Current American Heart Association BLS certification required.
- Minimum of one year of physical therapist experience in a health care facility or equivalent required; home care experience required.
- Demonstrated knowledge of physical therapy principles and strong verbal and written communication skills required.
- Current valid driver’s license, a good driving record, and reliable transportation required.
- Experience working within clinical policies and collaborating with interdisciplinary teams preferred.
- Facility type: Hospital-employed Home Health / VNA Rehabilitation services.

- Shift details: Days 0800-1630.
- Assignment focus: Home health visits providing physical therapy in patient residences following plan of care.
- Reporting structure: Reports to VNA Clinical Team Manager, Rehabilitation.
