- Location: Bel Air, MD (Harford County) — occasional float to Aberdeen within a 10-mile radius
- Attire: Business casual
- Holidays: Required as scheduled
- Weekends: Not required (available if interested)
- On-call: Not required
- Minimum 3 years' experience in inpatient hospital or acute care case management (first-time travel RNs with strong relevant experience considered)
- Current Registered Nurse (RN) license in Maryland (Social Workers with appropriate credentials may be considered)
- BLS certification preferred
- EPIC electronic charting experience preferred
- Ability to float between Bel Air and Aberdeen facilities as needed
- Coordinate discharge planning and ensure safe transitions of care, including placement and medical home care
- Manage an average daily census of approximately 15 inpatient cases
- Provide comprehensive care coordination and wrap-around services addressing social determinants of health
- Collaborate with interdisciplinary teams (physicians, nurses, social workers, case coordinators) to meet patient needs
- Document care plans and progress using EPIC and other clinical systems
- Maintain professional communication with patients, families, and community resources
