- Shift: Monday – Friday, 8:00 AM – 5:00 PM (Day Shift, 8-hour shifts)
- Hours Per Week: 40
- Orientation: 16 hours
- Mileage Paid
- Travel assignment: Locals not accepted — must live 75+ miles from facility
- Active Registered Nurse (RN) license for the U.S. (pending license accepted; final RN license must be obtained prior to start)
- Minimum 1 year of recent Home Health RN experience (2 years preferred)
- BLS certification required
- OASIS assessment knowledge required — experience with OASIS documentation in home health nursing
- EPIC charting experience preferred (recent electronic charting experience highly valued)
- Compliance documents due 5 business days prior to start
- COVID-19 vaccine required (medical/religious exemptions considered)
- Must pass Cardiac Dysrhythmia Test at orientation
- All requested time off must be included at submission
- Provide comprehensive home health nursing care to adult and geriatric patients in their homes as a Homecare RN
- Complete approximately 6 patient visits per day, managing diagnoses including COVID, CHF, DM, COPD, musculoskeletal conditions, CVA, behavioral health, and more
- Perform wound care and wound V.A.C. management, IV infusions, tracheostomy care, and medication administration
- Care for and manage peripheral IVs, central lines, ports, and PICC lines
- Use OASIS for patient assessment and documentation; maintain accurate electronic charting (EPIC experience preferred)
- Collaborate with interdisciplinary teams including Physical Therapy, Interpretation Services, and Social Services to coordinate patient-centered care
- Float within scope as needed to meet facility and patient needs
- Work in a supportive home health environment with opportunities to expand clinical skills in wound care, IV therapy, and complex case management
