LiveOnNY is currently looking for a Full-time Patient Navigator - Organ Donation!

Do you possess the attributes to be a successful Patient Navigator- Organ Donation and perform the following essential functions?

Responsibilities:

The FSA provides on-site responses to assess family dynamics and leads family donation conversations by advocating for donation and supports families through the donation process.
 •  Delivers high-quality bereavement support to potential donor families.
 •  Facilitates the opportunity for donation by keeping hospital staff informed regarding the plans and progression of the referral process.
 •  Offers donation in an empowering and inspiring manner. Responsible for educating potential donor’s Next-of-Kin (NOK) about donation options, status of donor designation, or requesting authorization for donation from legal NOK
 •  Obtains authorization or disclosure with first person authorization and conducts a uniform donor risk assessment questionnaire (UDRAI) with appropriate NOK/surrogate of the potential organ donor.
 •  Assess families understanding of grave prognosis and brain death declaration.
 •  Supports donor families by attending OR in Donation After Circulatory Death Donation (DCDD).
 •  Works in strong collaboration with hospital personnel, LiveOnNY’s Family Service Lead (FS Lead), LiveOnNY’s Administrator-on-Call (AOC), Referral Administrator (RA), and clinical staff to evaluate and determine the best time to discuss the opportunity for donation with potential donor families.
 •  Completes accurate, and timely documentation of donor-related activities in computerized confidential donor record, and forms according to established policies and procedures.

Qualifications:

Do you have the education and experience to be a Patient Navigator- Organ Donation?
 •  Bachelor’s degree preferred not required
 •  Relevant working experience: Patient Services/Relations or Navigator, Sales, Motivational Interview Training, Child Life Specialist, EMT/Paramedic, Customer Service Representative, Crisis Mangement, Substance Abuse Counselor, Health Administrator/ Community Case worker/ Social worker, Nursing-LPN, Educator/Teacher.
 •  Certification or license in nursing, social work, grief counseling or crisis intervention preferred.

This role requires frequent travel (valid driver license and own vechicle) and ability to commute to different hospital partners within our service area

Salary Range: $70,000-$$103,000
