Position Type: Contract
Schedule: 09:00am-05:00pm (includes a 60 min break)
Location: New York, NY
Responsibilities:
Provides social work services to patients and families to assist them in continuity of care planning and in enhancing their capabilities of coping with illness and daily functioning.
Collaborates with Hospital, New York-Presbyterian System, and community-based personnel to ensure efficient service delivery and a high level of customer satisfaction.
Requirements:
Licensed Masters Social Worker (LMSW) by the State of New York required.
2+ years in acute care medical social work experience, including hospital discharge planning
EPIC experience.
NICU exp a MUST.
Assisting 2 Care managers and 1 1/2 SW's.
Discharge and Safety planning
At least 1 year of hospital experience.
Master's Degree in Social Work (MSW) required.
Communication skills and Knowledge of Psychosocial Assessment required
