Lead Case Manager

The Lead Case Manager is responsible for the safe and timely release of minors in care and the day-to-day operations of the case management department, including the timely submission of reunification packets, service plans, and other assessments.
The ideal candidate is organized, adapts well to change, and has an efficient work schedule that is subject to work extended hours, weekends, holidays, and be on-call.

Roles & Responsibilities:

 •  Assign cases upon youth’s arrival.
 •  Ensure youth are properly enrolled into the program’s internal intake system.
 •  Perform weekly audits of the program’s active case files to ensure compliance.
 •  Review all assessments and Family Reunification Packets for accuracy of information.
 •  Serve as the main point of contact for approving initial placements, youth transfers, and information requests.
 •  Ensure all physical and web-based case files are properly closed upon youth departure.
 •  Prepare and submit weekly case log reports to program staff, local funding source representative and other entities.
 •  Maintain a daily updated list of the status for all cases to ensure adequate progress is being made.
 •  Carry a small caseload for youth that are identified as being high profile or complex cases.
 •  Coordinate with local pro bono attorneys for the timely provision of “Know Your Rights” presentations to youth.
 •  Facilitate attorney to client contact as requested by youth.
 •  Conduct initial intake interviews to include gathering familial, and possible sponsorship information.
 •  Must assist in the evacuation of youth as needed due to inclement weather conditions, natural disasters, or other unforeseen occurrences.
 •  Work independently, react to change productively, and handle other essential tasks as assigned while exercising a high level of confidentiality.

Basic Qualifications:

 •  Master’s degree in behavioral science, human services, or social services

OR
 •  Bachelor’s degree in behavioral science, human services, or social services with minimum 3 years progressive employment experience with supervisory and Case Management experience.
 •  One year experience working with youth, preferably in an ORR setting.
 •  Must be at least 21 years of age at the time of hire.
 •  Proficiency in English and Spanish (verbal and written).
 •  Possess a valid Driver’s License.
 •  Possess a clear Tuberculosis test.
 •  Ability to successfully clear a background check.
 •  Ability to successfully clear a drug screening.
 •  Provide immunization documentation for Tdap, Varicella, MMR, Hepatitis A and B, Varicella.
 •  Must be authorized to work in the U.S. for any employer.
