Pharmacy Resident, PGY2 Ambulatory Care, 2024/2025

PURPOSE

The purpose of the Henry J. Austin Health Center (HJAHC) PGY2 Ambulatory Care Residency Program is to build on Doctor of Pharmacy (PharmD) education and PGY1 pharmacy residency programs, or equivalent experiences, in order to prepare pharmacists to be highly qualified independent ambulatory care practitioners within Federally Qualified Health Centers or similar patient care settings.
These pharmacists will be prepared to take and pass the BCACP. These pharmacists will be integral members of a multidisciplinary primary care team skilled at educating patients and healthcare professionals, experienced at working within an integrated behavioral healthcare model, and prepared for leadership roles within their practice.
These pharmacists will improve their clinical research skills, present a poster at a professional conference, and will create a publication plan for their research. These pharmacists will develop the necessary teaching skills for clinical faculty positions.

PROGRAM DESCRIPTION

This 12-month* PGY2 Ambulatory Care residency program provides advanced clinical training to prepare the resident to become a skilled practitioner that is able to work as part of a multidisciplinary healthcare team.

This program will offer the resident the opportunity for a longitudinal primary care experience across integrated behavioral healthcare practice sites at HJAHC as well as a longitudinal outpatient pharmacy experience at the HJAHC Pharmacy Department.

The resident will interact directly with patients by providing Chronic Care Management and Comprehensive Medication Management and will collaborate with physicians, nurses, social workers, and other clinical staff. A focus will be on developing and refining skills to manage complex chronic medical conditions, patient-centered care, motivational interviewing, and primary literature evaluation.

Teaching experience is gained by serving as a co-preceptor for Advanced Pharmacy Practice Experience (APPE) students on rotation at HJAHC, creating and delivering educational seminars to healthcare professionals at HJAHC, and by lecturing at the Ernest Mario School of Pharmacy (EMSOP), Rutgers, the State University of New Jersey.
Additionally, the resident will complete a Teaching and Learning Certificate program available through EMSOP.

ESSENTIAL FUNCTIONS

The resident will be a self-directed, independent, motivated learner guided by the Residency Program Director and the appropriate preceptor(s) for each learning experience, who:

Provides medication counseling to patients

Provides drug information to physicians, nurses, patients and other health care professionals

Documents interventions with health care personnel related to drug therapy

Observes for adverse drug reactions

Documents all medication incident reports that they may be responsible for or witness to

Performs drug utilization evaluations on medications

Participates in patient care huddles

Attends departmental staff meetings and educational seminars

Provides in service education to health care professionals

Maintains patient confidentiality

Provides pharmacokinetic monitoring of patients receiving specific drugs and ensures that serum concentrations are drawn

Responds to emergency codes

Oversees activities of medication administration of nurses

Performs related duties as required

PHYSICAL & WORK REQUIREMENTS

Requirements

EDUCATION & EXPERIENCE:

Must be a graduate from an ACPE-accredited Doctor of Pharmacy (PharmD) program.

Must be a citizen of the U.S.A., or hold a visa allowing for the completion of the residency year.

Must have a residency certificate from a pharmacy practice residency (PGY1) before enrolling in this advanced residency program.

Applicant will be required to take and pass the NJ Pharmacy Board Examination within 90-days of starting the residency program.

Must be proficient in basic math skills

Demonstrated ability to use Microsoft Office applications, including Microsoft Word, Outlook, and Excel
