Nurse Practitioner - Palliative Care

Overview:

Mercyhealth, an award-winning fully integrated health care delivery organization based in southcentral Wisconsin and northern Illinois, is seeking a Nurse Practitioner to join our Palliative Care team at Mercyhealth Hospital and Trauma Center in Janesville, WI.

Opportunity
 •  Join and collaborate with a palliative care physician, and other palliative care team members, in an active practice to deliver palliative care and related health care services to patients and families.
 •  Perform bedside palliative care patient/family evaluation.
 •  Assist with diagnosis, treatment, and management of acute and chronic health conditions.
 •  Consult with palliative care physician or designees as needed, inform primary physician of services provided and collaborate with other physicians as needed.
 •  Assist in all facets of care coordination for palliative care referrals.
 •  Hours are M-F 8:00am – 5:00pm
 •  No call expectations initially, but a call component may be added in the future

Qualifications
 •  Graduate of accredited NP program
 •  Current WI RN and APNP license
 •  Current DEA
 •  Other certifications as required
 •  Minimum of 5 years nursing experience; clinical nursing care in oncology, hospice, geriatrics, intensive care preferred.
