REPORTS TO: Director of Nursing Services (DON)

JOB SUMMARY: The licensed practical nurse is responsible for making decisions that are based on the individual's educational preparation and nursing experience, and for providing nursing services as ordered on patient plan of care. Reports problems/potential problems to RN/Case Manager.

DUTIES AND RESPONSIBILITIES:

Participation in the nursing process by providing input to the clients plan of care

Implementing the care plan as written

Evaluating effectiveness of care plan, make recommendations for revisions as appropriate.

Records all pertinent observations and treatments as per agency policies. Prepares clinical progress notes and provide appropriate documentation of service in keeping with agency policies and procedures.

Provide for the emotional and physical comfort and safety of the client

Perform nursing procedures for which he/she has been trained

Notify appropriate person about unusual symptoms and/or changes in conditions of clients

Participate in case conferences, team meetings and performance improvement activities as assigned

Provides proof of current license by the state. Provides necessary information to verify education and work experiences

Participates in in-service programs

Practices safely and competently within the job description

Complies with all state regulatory acts

Complies with all agency policies and procedures including HIPAA Privacy Rules

Provides documentation of high tech certification in appropriate; performs duties as qualified/authorized

Maintains patient/business confidentiality

QUALIFICIATIONS:

Must be a graduate of an approved school of nursing and currently licensed in the state of the HHA.

Must have one year current experience in accredited hospital or accredited nursing home within two years previous to employment.
