Clinical Pharmacist - Per Diem

Assists with quality improvement projects, such as Drug Use Evaluations, analysis of interventions etc as requested by Pharmacy Leadership.
The pharmacist reviews, interprets and verifies all medication orders and prescriptions received by the pharmacy. Additionally, the pharmacists compounds and dispenses medications and supervises pharmacy technicians and interns who do the same.
The pharmacist will also assist with various clinical activities including rounding, attendance at CODE/Trauma cases, reviewing Pyxis reports for cost saving interventions, and staffing the anticoagulation clinic.

Responsibilities:

 •  1. Competent in the Practice of Pharmacy:
 •  1.1 Does not have high error rate when entering orders, making IV medications or checking the Pyxis refill.
 •  2. Controlled Substances Inventory and Record Keeping:
 •  2.1 Dispenses narcotics/controlled substances according to hospital policy.
 •  3. Sterile Product Preparation
 •  3.1 Complies 100% of time with required clean room attire and cleaning procedures. In the event of a technician or intern not complying, immediate corrective action is taken.
 •  4. Professional Conduct and Leadership
 •  4.1 Develops effective working relationships with pharmacy staff, nursing staff, medical staff and administration. Maintains positive attitude with respect to all hospital and departmental goals and objectives.
 •  5. Supports Hospital Information Systems
 •  5.1 Acts as resource for nursing, pharmacy and other staff for issues related to Pyxis or the pharmacy information system.
 •  6. Education and Safety Compliance
 •  6.1 Attends all required safety and educational programs.
 •  7. Clinical Practice
 •  7.1 Documents and completes all interventions, maintaining an adequate number each month as determined by the Director of Pharmacy.
 •  8. Operational Excellence
 •  8.1 Supports hospital and departmental efforts to achieve and maintain accreditation with all applicable regulatory bodies including DNV, RI DOH and ISO 9001.

Qualifications:

EDUCATION

Bachelor of Science in pharmacy or Doctor of Pharmacy degree from an accredited school.

EXPERIENCE

Minimum of one year experience as a pharmacist, with hospital experience preferred.

LICENSURE

BLS, ACLS, and PALS certification strongly encouraged.
