APN Healthcare Solutions is urgently hiring multiple for our client in Portland, OR for a 12/13 week contract with a high possibility of extension.
Position Title: Licensed Practical Nurse – LTC
Pay Rate: $40 - $42/Hourly
Work Location: Portland, OR
Shift: Night 3x12-Hour (19:00 - 07:00)
Hours/day: 12 Hours
Contract Duration: 13 Weeks
Licenses Requirement: LPN State License
Certification Requirement: BLS
Traveler's permanent address cannot be local within 50 miles of the facility.
Job Description:
Administer medications and treatments according to physician orders and facility protocols.
Monitor residents' health status and report any changes to the registered nurse or physician.
Assist residents with activities of daily living, including bathing, dressing, and toileting.
Collaborate with the interdisciplinary team to develop and implement individualized care plans for residents.
Provide emotional support and companionship to residents, promoting their overall well-being.
Document all nursing care provided and communicate effectively with team members.
Ensure a safe and clean environment for residents, adhering to infection control protocols.
Participate in resident care conferences and family meetings as required.
Respond to emergencies and provide appropriate interventions as needed.
Maintain licensure and pursue ongoing education to enhance nursing knowledge and skills.
