- Lead Patient Intakes: Collect and organize referral and medical documentation with precision.

- Maintain Accurate Records: Enter and manage patient information in EMRs and digital systems.
- Stay Compliant: Uphold HIPAA and privacy standards at all times.
- Support Patients & Families: Communicate clearly, professionally, and compassionately.
- Education: High school diploma required; associate’s or bachelor’s preferred.
- Experience: Minimum 2 years in healthcare — Home Health/Hospice intake experience required.
- Skills & Knowledge: Proficient in EMR systems, medical terminology, and insurance procedures.

- Strong Communicator: Confident, compassionate, and effective with patients, families, and providers.
- Adaptable: Comfortable managing shifting priorities and occasional evening/weekend needs.
