Registered Nurse RN Home Health | 1:1 Patient Care
You became a nurse to care for people.Not to rush through five patients at once.

- Complete patient assessments and reassessments
- Develop and update individualized care plans
- Perform start of care and follow up visits
- Communicate changes in patient condition with physicians and the care team
- Collaborate with an interdisciplinary team to improve patient outcomes
- Active RN license
- 1+ year of clinical nursing experience
- Ability to travel within the service area
- Valid driver’s license and reliable transportation
- Home Health or Hospice experience preferred
- HomeCare HomeBase experience preferred
