Job Overview
A reputable healthcare facility is seeking a compassionate and experienced Registered Nurse (RN) to join its Long-Term Care/Skilled Nursing team. This role focuses on delivering high-quality, patient-centered care to residents while collaborating with interdisciplinary healthcare professionals in a supportive clinical environment.
Key Responsibilities- Administer medications and treatments in accordance with physician orders and nursing standards
- Conduct comprehensive nursing assessments, including admissions and ongoing evaluations
- Develop, implement, and update individualized care plans
- Monitor resident conditions and document progress, treatments, and responses accurately
- Communicate effectively with residents, families, physicians, and care teams
- Assist residents with Activities of Daily Living (ADLs), including mobility, hygiene, and personal care
- Supervise and support nursing assistants and ancillary staff as needed
- Maintain accurate clinical documentation and ensure regulatory compliance
- Participate in shift reports, care coordination, and interdisciplinary meetings
- Follow infection control, safety, and healthcare compliance protocols
- Support staff education, training, and quality improvement initiatives
- Apply geriatric nursing principles to support residents with complex healthcare needs
- Active Registered Nurse (RN) license in the state of Arizona
- Strong knowledge of long-term care and geriatric nursing practices
- Experience with medication administration and patient treatment procedures
- Ability to perform assessments and develop effective care plans
- Knowledge of state, federal, and facility healthcare regulations
- Excellent communication, documentation, and interpersonal skills
- Ability to work collaboratively in a team-oriented environment
- Minimum 1 year of experience in a Skilled Nursing Facility (SNF) or Long-Term Care (LTC) setting
- Experience with MDS documentation and care planning
- Familiarity with EMR/EHR systems
- Experience working with geriatric or veteran patient populations
