Location: Yuma
Schedule: 12-Hour Day Shifts (6:00 AM – 6:30 PM)

Hours: 36–40 Hours per Week

Job Overview

A reputable healthcare facility is seeking a compassionate and experienced Registered Nurse (RN) to join its Long-Term Care/Skilled Nursing team. This role focuses on delivering high-quality, patient-centered care to residents while collaborating with interdisciplinary healthcare professionals in a supportive clinical environment.

Key Responsibilities
  • Administer medications and treatments in accordance with physician orders and nursing standards
  • Conduct comprehensive nursing assessments, including admissions and ongoing evaluations
  • Develop, implement, and update individualized care plans
  • Monitor resident conditions and document progress, treatments, and responses accurately
  • Communicate effectively with residents, families, physicians, and care teams
  • Assist residents with Activities of Daily Living (ADLs), including mobility, hygiene, and personal care
  • Supervise and support nursing assistants and ancillary staff as needed
  • Maintain accurate clinical documentation and ensure regulatory compliance
  • Participate in shift reports, care coordination, and interdisciplinary meetings
  • Follow infection control, safety, and healthcare compliance protocols
  • Support staff education, training, and quality improvement initiatives
  • Apply geriatric nursing principles to support residents with complex healthcare needs
Required Qualifications
  • Active Registered Nurse (RN) license in the state of Arizona
  • Strong knowledge of long-term care and geriatric nursing practices
  • Experience with medication administration and patient treatment procedures
  • Ability to perform assessments and develop effective care plans
  • Knowledge of state, federal, and facility healthcare regulations
  • Excellent communication, documentation, and interpersonal skills
  • Ability to work collaboratively in a team-oriented environment
Preferred Qualifications
  • Minimum 1 year of experience in a Skilled Nursing Facility (SNF) or Long-Term Care (LTC) setting
  • Experience with MDS documentation and care planning
  • Familiarity with EMR/EHR systems
  • Experience working with geriatric or veteran patient populations
