Shift Options (Specify Preference):
- Day Shift: 6:00 AM – 6:30 PM
- Night Shift: 6:30 PM – 6:00 AM
Position Overview
"Navitas Healthcare, LLC" is seeking a skilled Licensed Practical Nurse (LPN) to provide high-quality patient care in a structured healthcare setting. The LPN will support residents through medication administration, treatments, and coordination with the healthcare team while ensuring compliance with state and facility standards.
Key Responsibilities- Administer medications (oral, enteral, parenteral, etc.)
- Perform treatments (wound care, trach care, catheter care, dressing changes)
- Monitor patient response and report changes in condition
- Maintain medication and supply availability
- Participate in shift handoffs and patient reporting
- Communicate with residents, families, and care teams
- Conduct initial assessments for new admissions
- Assist with MDS, care planning, and reviews
- Maintain accurate documentation of patient care
- Participate in QAPI and quality improvement initiatives
- Active Arizona LPN License
- Strong clinical judgment and emergency response ability
- Ability to assess, document, and report patient conditions
- Strong communication and teamwork skills
- Experience in geriatric or long-term care settings
- Familiarity with care planning and MDS processes
