• Day Shift: 6:00 AM – 6:30 PM
• Night Shift: 6:30 PM – 6:00 AM

Position Overview

"Navitas Healthcare, LLC" is seeking a skilled Licensed Practical Nurse (LPN) to provide high-quality patient care in a structured healthcare setting. The LPN will support residents through medication administration, treatments, and coordination with the healthcare team while ensuring compliance with state and facility standards.

• Administer medications (oral, enteral, parenteral, etc.)
• Perform treatments (wound care, trach care, catheter care, dressing changes)
• Monitor patient response and report changes in condition
• Maintain medication and supply availability
• Participate in shift handoffs and patient reporting
• Communicate with residents, families, and care teams
• Conduct initial assessments for new admissions
• Assist with MDS, care planning, and reviews
• Maintain accurate documentation of patient care
• Participate in QAPI and quality improvement initiatives

• Active Arizona LPN License
• Strong clinical judgment and emergency response ability
• Ability to assess, document, and report patient conditions
• Strong communication and teamwork skills

• Experience in geriatric or long-term care settings
• Familiarity with care planning and MDS processes
