Community Health Nurse, Manhattan (Full Time)

Overview:

The Community Health Nurse provides comprehensive, client-centered nursing care in a community-based setting. Responsibilities vary by location and include conducting health assessments, administering medications, implementing physician orders, and coordinating care with onsite and community healthcare providers.

This role involves monitoring client health status, delivering first aid and emergency response as needed, and educating clients and staff on medication management, disease prevention, and Narcan use. The nurse also develops and oversees individualized care plans, ensures a safe and hygienic environment, and maintains accurate clinical documentation and data reporting.

Working collaboratively with program leadership and multidisciplinary teams, the Community Health Nurse plays a key role in promoting client health, safety, and continuity of care.

What You Will Do:

  • - Collaborates with interdisciplinary teams to coordinate health and wellness activities.
  • Conducts client assessments to determine health needs, including taking vital signs as needed.
  • Accurately documents assessments, interventions, and outcomes in clients’ medical records.
  • Provides emergency care until EMS arrives and liaises with EMS personnel during medical emergencies.
  • Develops and implement individualized nursing care plans.
  • Health Promotion: Educates clients on disease prevention and health maintenance.
  • Assists in preparing statistical reports for funding sources (e.g., number of health assessments and referrals to primary care or specialists).
  • Care Coordination and Advocacy: Facilitates referrals and linkages to appropriate healthcare services based on client needs.

Qualifications:

Licenses and Certifications:

  • License and current registration to practice as a Registered Professional Nurse in New York State required
  • CPR/BLS Certification required

Education:

  • Associate's Degree Nursing required Bachelor's Degree Nursing preferred

Work Experience:

  • Minimum two years of experience working as an RN
  • Prior experience working with behavioral health client population and providing clinical assessments of their health needs preferred

Pay Range: USD $85,000.00 - USD $106,300.00 /Yr.
