Overview:

VNS Health Community Health RN's serve as a member of an interdisciplinary care team within a NORC (Naturally Occurring Retirement Community) or senior living community, coordinating a broad range of health and social services to support older adults in aging safely in place.

NORCs achieve this by facilitating and integrating existing community-based health and social services, while identifying and organizing additional supports needed to help residents remain independent and engaged in their homes and communities.

What You Will Do:

  • Collaborates with interdisciplinary teams to coordinate health and wellness activities.
  • Conducts client assessments to determine health needs, including taking vital signs as needed.
  • Accurately documents assessments, interventions, and outcomes in clients’ medical records.
  • Provides emergency care until EMS arrives and liaises with EMS personnel during medical emergencies.
  • Develops and implement individualized nursing care plans.
  • Health Promotion: Educates clients on disease prevention and health maintenance.
  • Assists in preparing statistical reports for funding sources (e.g., number of health assessments and referrals to primary care or specialists).
  • Care Coordination and Advocacy: Facilitates referrals and linkages to appropriate healthcare services based on client needs.

Qualifications:

Licenses and Certifications:

  • License and current registration to practice as a Registered Professional Nurse in the State of New York required
  • CPR/BLS Certification required

Education:

  • Associate’s degree in nursing from an approved program required
  • Bachelor’s degree in nursing preferred

Work Experience:

  • Minimum of two years’ clinical home care experience

Pay Range: USD $40.95 - USD $51.19 /Hr.
