Licensed Master Social Worker (LMSW) - 26-05025

Job Location: New York, NY

Day Shift (3x8-hour schedule) with variable timings:

• Monday: 9:30 AM – 4:00 PM
• Tuesday: 9:00 AM – 5:00 PM
• Wednesday: 10:00 AM – 3:00 PM

Job Duration: 27 Weeks

This position will cover a full-time equivalent leave of absence.

Responsibilities:

• Perform detailed psychosocial assessments for patients
• Develop and implement care plans in coordination with healthcare providers
• Provide emotional support, counseling, and resource guidance to patients and families
• Collaborate with interdisciplinary teams to optimize patient outcomes
• Maintain accurate and timely documentation

Requirements:

• Master’s Degree in Social Work (MSW) – required
• Licensed Master Social Worker (LMSW) in the State of New York (must obtain within 1 year of hire)
• Strong communication and interpersonal skills
• Knowledge of psychosocial assessment techniques
• Experience in a hospital or outpatient medical setting preferred
• Transplant experience is a plus but not required
