Licensed Master Social Worker (LMSW) – Medical Social Services - New York, NY

• Monday: 9:30 AM – 4:00 PM
• Tuesday: 9:00 AM – 5:00 PM
• Wednesday: 10:00 AM – 3:00 PM

Position Overview

Experience in transplant services is a plus but not required.

• Conduct comprehensive psychosocial assessments for patients and families
• Develop and implement individualized care and discharge plans
• Provide emotional support, counseling, and crisis intervention
• Collaborate with multidisciplinary healthcare teams to coordinate patient care
• Connect patients with community resources and support services
• Advocate for patient needs and ensure continuity of care
• Maintain accurate and timely documentation in compliance with regulatory standards
• Support patients with chronic illness, complex conditions, or transplant-related care
• Participate in case conferences and interdisciplinary team meetings

• Master’s Degree in Social Work (MSW) – Required
• Active Licensed Master Social Worker (LMSW) license in the State of New York (or ability to obtain within one year of hire) – Required
• Prior experience in a hospital or outpatient medical setting – Required
• Strong knowledge of psychosocial assessments and care planning
• Excellent communication, interpersonal, and organizational skills
• Ability to work effectively within a multidisciplinary team
• Experience in transplant social work – Preferred
