Position Summary
"Navitas Healthcare, LLC" is seeking a dedicated Licensed Practical Nurse (LPN) to provide high-quality, patient-centered care in a long-term care setting. This role involves direct resident care, supervision of nursing assistants, and collaboration with interdisciplinary teams to ensure compliance with all regulatory standards and best practices.
Key Responsibilities- Supervise and direct nursing assistants and support staff during assigned shifts
- Administer medications and treatments as prescribed
- Transcribe and implement physician orders into care plans and documentation systems
- Monitor resident conditions and maintain accurate nursing documentation
- Perform clinical procedures such as catheterization, tube feeding, wound care, and suctioning
- Collect specimens (urine, blood, sputum) and assist with routine lab tasks
- Record vital signs and track patient progress
- Coordinate admissions, transfers, and discharges, including required documentation
- Communicate effectively with physicians and healthcare team members
- Participate in care planning and interdisciplinary collaboration
- Ensure availability of supplies, medications, and equipment
- Educate residents and families on care plans and treatments
- Maintain HIPAA compliance and resident confidentiality
- Assist during facility surveys and inspections
- Provide compassionate end-of-life care and support
- Experience in long-term care or skilled nursing – Preferred
- Strong medication administration and clinical skills – Required
- Supervisory experience with nursing assistants – Required
- Excellent communication and documentation skills – Required
- Ability to prioritize and manage multiple patients effectively
- Graduate of an accredited nursing or LPN program – Required
- Active, unencumbered Massachusetts LPN License – Required
- BLS / CPR / AED Certification – Required
