Unit Overview:
- Number of Beds: 350
- Staff Composition: Case Managers (CM), Social Workers (SW), and SNF Coordinator
- Shift Coverage:
- Day Shift: As per unit structure (e.g., 1 charge, 3 RN, 2 CNA, 1–3 sitters)
- Night Shift: N/A
- Patient Ratio: 25:1
- Equipment: N/A
- EMR Systems: Meditech and Midas
Key Requirements (Must Have):
- Proven acute care case management (CM) experience
- Ability to manage a patient ratio of 25:1 in a fast-paced environment
- Strong understanding of case management workflows, discharge planning, and care coordination
- Ability to work independently while collaborating with interdisciplinary teams
- Strong communication, organizational, and time management skills
Preferred Qualifications:
- Experience with Meditech and Midas documentation systems
- Prior HCA facility experience
Core Competencies:
- Effective patient care coordination and discharge planning
- Accurate and timely documentation
- Collaboration with CM, SW, and SNF coordination teams
- Ability to prioritize and manage multiple cases efficiently
Scheduling & Work Expectations:
- Shift: 8:00 AM – 5:00 PM
- Travelers will be scheduled in accordance with facility policy
- Weekend Requirement: Every other weekend
- Holiday Requirement: Rotational, in alignment with hospital staff
