Job Summary
The Pharmacist is responsible for preparing, dispensing, and monitoring medications to ensure safe and effective pharmaceutical care for patients in an acute care hospital setting. The role includes reviewing medication orders, providing clinical consultation, maintaining compliance with regulatory standards, and collaborating with physicians, nurses, and healthcare staff to promote optimal patient outcomes.
Key Responsibilities- Review and verify medication orders for accuracy, appropriateness, and safety.
- Dispense medications in accordance with physician orders and established pharmacy policies.
- Prepare and oversee sterile and non-sterile compounded medications, including IV preparations when required.
- Monitor patient drug therapies and evaluate for drug interactions, allergies, and contraindications.
- Provide drug information and clinical consultation to physicians, nurses, and other healthcare professionals.
- Ensure compliance with federal, state, and hospital regulatory requirements.
- Maintain accurate medication records within pharmacy dispensing systems and EMR platforms.
- Participate in medication safety initiatives and quality improvement activities.
- Manage medication inventory, storage, and controlled substances according to regulatory standards.
- Support pharmacy operations including IV room workflow and sterile processing procedures.
- Minimum one (1) year of Pharmacist experience.
- Strong knowledge of pharmacology, medication safety, and clinical pharmacy practices.
- Ability to work effectively in a fast-paced acute care environment.
- Excellent communication and interdisciplinary collaboration skills.
- Experience with pharmacy dispensing systems.
- Sterile processing and IV room experience.
- Knowledge of Electronic Medical Records (EMR) systems.
Education
Required:
- Bachelor's degree or higher in Pharmacy.
Certifications & Licensure
Required:
- Valid New York State Pharmacist License issued by NYSED (Primary Source Verification required).
